Abstract

An examination of the dying process as it is experienced in painful and debilitating diseases from the point of view of the sufferers and their families. The author considers the idea of assisted suicides, and also reflects on religious, moral and legal issues involved in someone's death. This book is written for all those who are concerned about how their life may end - and who wish to die without unnecessary suffering. Dr. Charles F. McKhann discusses many aspects of physician-assisted dying and explains why he thinks it should be made legally available under certain circumstances. Dr. McKhann, a specialist in cancer surgery, has conducted in-depth interviews with people who were dying form a variety of illnesses and with the physicians who cared for them. Drawing on these interviews and on his own experiences as a physician, he looks at the dying process as it is encountered in painful and debilitating diseases, and he considers the needs of patients and their families. Dr. McKhann presents the case for rational suicide, comparing a failed suicide attempt in the United States with a planned death in the Netherlands and illustrating the differences in approach and attitudes. He describes the forms of physician assistance already taking place and acknowledges the physician's personal and professional concerns. And he reflects on relevant religious, moral, legal, and public-policy issues that are currently so widely debated. His thought-provoking book is a valuable resource not only for the general public but also for compassionate physicians who attend people with fatal diseases and for lawmakers who strive for understanding and courage in dealing with this new challenge.

Abstract

Significantly, questions about what physicians should do in the face of demands for such treatments have arisen more frequently now, in this era of health care competition, managed care, and increased focus on costs and the bottom line. But despite pressures to set limits, many of them legitimate, Rubin argues that the futility debate, and the policies and practices that have evolved in response to it, are misguided. She rejects the popular arguments supporting unilateral decision making by physicians, and calls instead for a different kind of conversation about the central values at stake when doctors and patients so dramatically disagree. The medical situation is critical, even life-threatening. The doctor refuses to offer or to continue providing a medical treatment. The patient, or the patient's family, insist that everything possible must be done. The doctor says no, such a treatment is futile. In the face of such a conflict, what should be done and who should decide? Should physicians be permitted to unilaterally refuse to provide treatment that they deem futile? In this book, philosopher and bioethicist Rubin examines this controversial issue. She offers a critique of the concept of medical futility and the debate surrounding it, and she calls for more public debate about the underlying issues at stake for all of us - patients, families, health care providers, insurers, and society at large.For many years, the field of bioethics was preoccupied not with the physician's but with the patient's right to say no to unwanted medical treatment. Today the tables have turned and it is the health care professionals who want to refuse medical treatments they deem futile.

Abstract

Cultural forces shape much of medicine including psychiatry, and medicine shapes much of our culture. Medicine provides us with beneficial treatments of disease, but it also causes harm, increasingly so in the form of overmedication enhanced by the pharmaceutical industry. The book explores boundaries of medicine and psychiatry in a cultural setting by building bridges between unconnected literatures. Boundaries have to be redrawn since effects of the environment, biological, social and political, on health and disease are undervalued. Potential beneficial effects of diet therapies are a recurrent theme throughout the text, with particular emphasis on omega-3 fatty acids. Deficiencies of these acids in common diets may contribute to many chronic diseases and psychiatric disorders. The book uncovers limitations of evidence-based medicine, which fosters a restrictive view of health and disease. Case studies include: the biology of migraine; limitations of biological psychiatry; conventional versus alternative medicine; science, religion and near-death experiences.

I do so as both a physician and a philosopher. Of my two voices, it is the latter that is informed by the former ... As a physician I have sought professional solutions to the frustrations of fighting a medical system that has become increasingly hostile to my standards of care for my patients; as a philosopher I will explore here the ethical issues I believe are the root of our predicament."--The introduction. In Confessions of a Medicine Man, Alfred Tauber probes the ethical structure of contemporary medicine in an argument accessible to lay readers, healthcare professionals, and ethicists alike. Through personal anecdote, historical narrative, and philosophical discussion, Tauber composes a moral portrait of the doctor-patient relationship. In a time when discussion has focused on market forces, he seeks to show how our basic conceptions of health, the body, and most fundamentally our very notion of selfhood frame our experience of illness. Arguing against an ethics based on a presumed autonomy, Tauber presents a relational ethic that must orient medical science and a voracious industry back to their primary moral responsibility: the empathetic response to the call of the ill.
